Birth Certificates

Beginning Tuesday, July 6, 2021 — Hours (By Appointment Only):

Monday – Friday   8:30 am – 11:00 am & 1:00 pm – 2:30 pm

The Dorchester County Health Department can issue official birth certificates with a raised seal for anyone born in the State of Maryland.  Birth certificates can only be issued to the person or the parent named on the certificate. Legal guardians must present original Court documents.

Please call 410-228-3223 to schedule an appointment time within the timeframes given above.  Please bring the following items with you:

      A: Valid, unexpired ID** (cannot be damaged):

  • State issued drivers’ license
  • State issued identification card (MVA)
  • Current U.S. Passport 
  • Current passport issued by another country (Must have issue date and expiration date)

*Mail with the applicant’s name and current address must accompany the passport and also if the address is not current on State issued drivers’ license/ID.

**PLEASE NOTE: As Maryland continues its COVID-19 recovery, Governor Larry Hogan’s Executive Order issued on May 12, 2021 allows customers who have an MDOT MVA product that expired since March 12, 2020 to have those products renewed under the following deadlines: All driver’s licenses – both non-commercial and commercial (CDLs) – as
well as learner’s permits and identification (ID) cards that expired during the state of emergency must be renewed by August 15, 2021. Therefore, an expired State issued ID with an expiration date between March 12, 2020 and August 15, 2021 will be accepted.

     B: Fee is $25.00 per certificate. Cash (exact change preferred) or credit cards (Master Card,
VISA, Discover) are accepted as payment.

     C: Completed Application for Certified Copy of Maryland Birth Record.  Please click here to download, complete, print and bring with you.  If you do not have access to a computer and printer, you will be given the application to complete when you arrive.

Upon arrival for your appointment, please enter the building at the front entrance and report in to the Front Desk Receptionist to let her know that you have arrived.  All visitors coming into the building must be screened.  Screening consists of temperature check and a questionnaire.  All visitors are required to wear a mask while in the Front Lobby of the building.

Death Certificates

At this time the Dorchester County Health Department is issuing death certificates to funeral directors only.   Please contact the funeral establishment that handled the arrangements for the deceased or the Maryland Division of Vital Records at  www.health.maryland.gov/vsa
